Require a supplied decision and a selected new output filename.

If the decision is missing, ask for it; do not infer agreement from discussion.

Read an existing destination before revising it. Preserve unrelated records.

Write three headings: Decision, Reason, Open questions.

Preserve supplied decision wording and all numbers. Do not invent an owner or date.

Use “Not supplied” for an absent reason or absent open questions.

Save the Markdown file with actual file tools and read it back.

Do not contact attendees, approve the decision or publish the record.

Example: “Use a checklist; reason: avoid missed steps; open: who reviews it?” becomes those three supplied facts under the three headings.

Self-test one: record that example and compare all three facts.

Self-test two: input has only a reason. Ask for the missing decision and do not create a false decision record.
